This form gives parent permission for publication of the student's photo. In accordance with district policy, it is required that we obtain parental permission before any recognizable student photo is to be published online. |

This is to provide parents and students with information on the district's athletic program. Please read and have a parent and the student sign the form. We require two copies to be returned to the school and it is also recommended that an addition copy be made for your records. |

This is for paying the different fee's to participate in an athletic activity or program. There is also a space to request a waiver of the fee, if necessary.
